The Assistant Social Worker – Mental Health supports the delivery of psychosocial and social care services within a mental health setting under the supervision of a licensed Social Worker or Mental Health professional. The role focuses on assisting patients, families, and caregivers with social, emotional, and practical support needs, contributing to continuity of care, patient safety, and recovery‑oriented services.
Responsibilities
- Support licensed Social Workers in conducting basic psychosocial assessments, care coordination, and follow‑up for mental health patients.
- Assist patients and families in accessing community resources, social support services, and welfare programs as part of the treatment and discharge planning process.
- Provide emotional and practical support to patients in inpatient, outpatient, or community mental health settings, ensuring dignity, confidentiality, and cultural sensitivity.
- Participate in multidisciplinary mental health team meetings and contribute to coordinated care plans under supervision.
- Maintain accurate documentation, case notes, and records in line with hospital policies, DOH regulations, and accreditation standards.
- Support mental health programs, awareness initiatives, and patient education activities as assigned by the supervising Social Worker.
Qualifications
- Bachelor’s degree in Social Work.
- 0-2 years of experience in a healthcare, mental health, community, or social care setting (preferred)
