Requirements:

• Bachelor’s degree in Social Work
• Valid SCHS Classification & Registration
• Hospital experience is preferred
• Strong communication and interpersonal skills
  Responsibilities:
• Provide psychosocial support and counseling to patients and their families.
• Coordinate discharge planning with the multidisciplinary team.
• Connect patients with appropriate community resources and services.
• Manage deceased affairs and coordinate body release procedures in accordance
  with hospital policies and regulations.
• Maintain accurate documentation and patient records.
