Leadership
- Provides professional leadership and acts as a role model and mentor to fellow nursing staff
- Demonstrates critical thinking and provides leadership in conflict management and problem-solving
- Adopts an interdisciplinary team approach based on mutual respect and direct communication between health professionals to provide patient and family-centered care.
- Liaises with Nurse Manager/Nurse Supervisor to provide safety for the patient and nurse by ensuring patient care is addressed with the appropriate staffing ratio and skill mix
- Monitors clinical care to ensure clinical standards are met. Identifies variances and initiates corrective measures as required
- Monitors staff performance, providing regular constructive feedback, coaching, guidance, and support to team members on the shift. Reports to concerned person deviations from policy and procedure using the appropriate chain of command.
- Participates, as assigned, on committees, working groups, and other organization activities.
- Actively participates in organizational change in a constructive manner.
- May assume management of the unit in the absence of the Nurse Manager.
- In collaboration with the Nurse Manager, shares in the responsibility of formal evaluation of staff performance and achievement of annual performance goals
- Performs consistent, systematic evaluation of diabetes education and care issues to determine if they meet patient/staff needs and communicate with the nursing education department for further actions as needed
Clinical Practice
- Responsible for the assessment, planning, implementation, and evaluation of patient care/Diabetes education in accordance with the organization ’ s policy and procedures and the unit scope of service
- Responsible for timely and accurate documentation of patient care that is compliant with organizational policies. Assesses and facilitates staff knowledge and compliance with documentation .
- Facilitates and monitors compliance downtime procedures consistent with SEHA policy .
- Coordinates individualized, integrated plan of care from point of first contact throughout the episode of care, incorporating discharge planning and setting measurable goals that are transparent to the patient .
- Assumes accountability and responsibility for own/staff practice and does not carry out tasks beyond the level of competence. Monitors staff adherence to competencies .
- Promotes and utilizes evidence-based practice in the provision of care/diabetes education
- Prioritizes patient care/education based on ongoing assessment of patient needs and assists the team by prioritizing their workload where appropriate .
- Identifies significant changes in patient condition, supports staff nurse in the initiation of appropriate action, and reports to immediate supervisor or physician as appropriate. Utilizes early warning scoring tools to identify deterioration in a timely fashion. Reallocates patient assignment to allow for a higher level of care for the deteriorating patient on the unit where appropriate .
- Demonstrates knowledge in the assessment and reassessment of pain and complies with HCF policies, guidelines, and documentation in the treatments and escalation of pain management. Supports staff in an escalation of pain management where appropriate .
- Coordinates referrals and utilizes available resources for the provision of comprehensive care to patients and families
- Demonstrates service excellence and acts as a patient advocate to support the patient ’ s rights and dignity at all times .
- Facilitates a climate of mutual understanding, and appreciation of cross-cultural differences inherent in a multicultural society .
- Demonstrates a wide scope of knowledge and understanding of medication and high alert/narcotic policy and is involved in teaching and compliance monitoring .
- Is responsible to ensure that on a shift basis at the unit level, the monitoring, replacement, storage, and administration of Narcotic and Controlled Drugs are in accordance with the Ministry of Health and DOH Narcotic Policies and Procedures .
- Collaborates with Pharmacy services to ensure the provision of medication needs of the unit .
- Reports Adverse Drug Reactions (ADRs) and Medication errors in accordance with organization policy .
- Assists with monitoring of PSN reports and supports the actions required to mitigate recurrence .
- Guides response to all levels of emergencies according to the organization’s policies and procedures
- Provides guidance and advice to the patient care team on clinical, educational, and administrative matters .
Communication
- Liaises with other health care providers as necessary to ensure appropriate and timely interventions for patients and for discharge planning.
- Provides a comprehensive clinical handover / report using SBAR to ensure patient safety. Ensures staff handover is carried out according to HCF policy and procedure .
- Attends patient care rounds and communicates and updates the attending healthcare provider on changes in the patient ’ s condition .
- Communicates with patient care team ongoing changes in patient ’ s plan of care and unit activity
- Ensures the nurses communicate and report changes in the patient ’ s condition to the relevant health care provider and that new orders are carried out according to HCF policy and procedure .
- Monitors and audits compliance with organization documentation standards
- Establishes and maintains professional boundaries with patients and members of the healthcare team .
Performance Improvement/Risk Management
- Participates in and/or leads quality improvement activities by assisting with the development, implementation, and review of standards, policies, procedures, and guidelines. May assist with evaluation of indicators and reporting of outcomes and trends .
- Monitors and evaluates unit operation and services to identify problems and takes a lead role in improving systems and safe patient care
- Assists with environmental rounds identifies risks, escalates, and maintains records of reporting process when risks are identified .
- Provides education to staff, patients, and visitors regarding a safe environment .
- Participates in and assists with organizing mock code drills in the unit as appropriate .
- Promotes and audits compliance with the organization ’ s Infection Control policies and procedures
- Identifies, investigates, reports, and implements corrective action for incidents or complaints as appropriate in consultation with the Unit Manager or Nursing Supervisor. Completes appropriate documentation according to organization policy .
- Delegates responsibilities to the nursing team in response to an emergency .
- Demonstrates a wide scope of understanding of the JCIA and DOH international patient safety goals and tis involved in teaching and compliance monitoring .
- Complies with EHSMS standards as per DOH requirements
Patient and Family Education
- Assess patient and family education needs including psychological and behavioral health, develop an individualized education plan based on the patient’s assessed needs and goals, and promotes successful self-management
- Partners with patients to deliver care and education conducive to behavior change and improved quality of life for self-management of diabetes across the lifespan
- Provides quality diabetes self-management education for patients and families based on assessed needs and in a culturally sensitive manner.
- Utilizes appropriate teaching techniques that are sensitive to the learning preferences of the person with prediabetes or diabetes.
- Advocates for and supports technology-enabled diabetes education and care, in individual and population health diabetes services.
- Advocates for and communicates about improved quality of care and outcomes for those living with, at risk for, and affected by diabetes
- Plan, implement and evaluate patients’ group education and support group in a culturally sensitive approach
- Evaluate the effectiveness of educational services provided by measuring attainment of learning objectives and work on areas for improvement
- Ensures patients and families are provided with comprehensive information and resources to meet their health care needs.
- Provides a suitable environment that supports patient and family learning.
- Engages in health promotion activities in the healthcare organization and community.
- Collaborates with the multidisciplinary team in the planning, implementation, and delivery of the patient education process.
- Implement the appropriate educational theories/established practices to promote health and wellness.
Professional Responsibilities
- Practices within the scope of practice as defined by the regulatory body
- Adheres to the Code of Conduct for Abu Dhabi and the organization
- Identifies learning needs of the junior staff on the Unit in conjunction with the Clinical Resource Nurse and Manager and assists with the implementation of educational activities .
- Completes mandatory, unit-specific competencies and diabetes education competency and demonstrates ongoing competence. Acts as a resource for staff in regard to competency self-evaluation .
- Integrates skills and knowledge of pathophysiology, epidemiology, clinical management, and self-management of diabetes into clinical practice.
- Contributes to research and applies current research and evidence-based care to practice.
- Gets involved in ongoing reviews and up-dates of the patients’ and family diabetes educational materials
- In conjunction with, but not limited to, the Line Manager/CRN, identifies own professional learning needs and develops a learning plan as required
- Develops and maintains own Professional Portfolio
- Initiates or participates in clinical research that contributes to continual improvement in standards of care
- Obtains accredited and non-accredited Continuing Nursing Education (CNE) Units in compliance with DOH and the organization ’ s requirements
- Demonstrates evidence of ongoing professional development in leadership and management
QUALIFICATIONS
Qualification Required :
- Bachelor’s degree in Nursing OR
- Associate degree in Nursing from Canada or USA with a minimum two (2) years course duration AND
- Successful completion of the National exam :
- USA: NCLEX – RN
- Canada: Licensing Exam
Specialist Required :
Minimum 1-year postgraduate accredited specialized program.
Preferred :
- Certificate proof of yearly 20 CE related to Diabetes Education
- Continuous of post graduate courses
Experience Required :
- A total of 3 years’ experience post qualification inclusive of one year of team leader experience or acting charge nurse experience .
- UAE Nurses: total 2 years post qualification, inclusive of 6 months of team leader experience.
Preferred :
- Has or is working toward a relevant post-graduation qualification
- Participation in quality or organizational improvement projects
1 year in a specialty area
