Core Responsibilities: –

  • Advanced Care: Engage deeply in patient care, optimizing complex medication therapies and anticipating potential medication-related problems before they arise.
  • Documentation: Maintain meticulous patient medication records, ensuring that therapeutic insights, interventions, and outcomes are accurately captured for continuous care.
  • Mentorship: Provide guidance and support to junior pharmacy staff and students, facilitating their professional growth and ensuring a high standard of care.
  • Therapeutic Committees: Participate in therapeutic committees within the hospital, providing expert insights on medication selection, formulary decisions, and therapeutic guidelines.
  • Continuous Education: Stay updated with the latest pharmaceutical research, guidelines, and best practices. Disseminate this knowledge to the pharmacy team and the wider hospital staff as appropriate.
  • Patient Counseling: Offer advanced counseling to patients, particularly those on complex medication regimens, ensuring they understand their medications and are adherent to their regimens.
  • Introductory Antimicrobial Stewardship Involvement: Contribute to optimizing antimicrobial use by participating in routine regimen reviews, assisting in the application of institution-specific guidelines, supporting educational initiatives, monitoring basic resistance trends, and working collaboratively within the stewardship team.
  • Quality Assurance: Oversee and contribute to quality assurance processes within the pharmacy, ensuring that the department delivers consistent, high-quality care.
  • Protocol Development: Assist in the development, revision, and implementation of medication protocols and guidelines, ensuring they align with the latest evidence and best practices.

Qualifications

  • 1-2 years of clinical pharmacy experience.
  • Master’s in clinical pharmacy/Basic clinical PharmD.
