Job Description

Key Responsibilities:

Patient Care Support:

  • Provide direct patient care, including vital sign checks, blood glucose monitoring, and patient observations
  • Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans

Clinical Procedures Assistance:

  • Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards
  • Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection

Patient Education and Communication:

  • Communicate effectively with patients, providing empathy and clarity to address their questions and concerns
  • Reinforce health education provided by physicians, encouraging self-care practices and adherence to treatment plans

Documentation and Reporting:

  • Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR)
  • Report any changes in patient condition to the supervising nurse or physician promptly

Operational and Safety Compliance:

  • Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and well-being of patients and staff
  • Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support

Requirements

Qualifications:

  • Valid DHA Nurse License – A MUST
  • Minimum of 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management
  • Knowledge of diabetes management and patient education in lifestyle-related chronic conditions is preferred
  • Strong communication and interpersonal skills with a patient-centric approach
  • Basic proficiency in electronic medical records (EMR) systems is preferred
