Job Description
Key Responsibilities:
Patient Care Support:
- Provide direct patient care, including vital sign checks, blood glucose monitoring, and patient observations
- Educate patients on diabetes management, including lifestyle tips and monitoring instructions, ensuring compliance with care plans
Clinical Procedures Assistance:
- Prepare examination rooms and medical equipment, ensuring cleanliness and adherence to infection control standards
- Assist in the preparation and handling of equipment for procedures, wound care, and specimen collection
Patient Education and Communication:
- Communicate effectively with patients, providing empathy and clarity to address their questions and concerns
- Reinforce health education provided by physicians, encouraging self-care practices and adherence to treatment plans
Documentation and Reporting:
- Accurately record and update patient information, treatment plans, and observations in electronic medical records (EMR)
- Report any changes in patient condition to the supervising nurse or physician promptly
Operational and Safety Compliance:
- Adhere to all DHA regulations, nursing guidelines, and GluCare protocols to ensure the safety and well-being of patients and staff
- Participate in ongoing training and development programs to stay current with best practices in diabetes care and nursing support
Requirements
Qualifications:
- Valid DHA Nurse License – A MUST
- Minimum of 1-2 years of experience in a clinical setting, preferably in endocrinology, diabetes care, or chronic disease management
- Knowledge of diabetes management and patient education in lifestyle-related chronic conditions is preferred
- Strong communication and interpersonal skills with a patient-centric approach
- Basic proficiency in electronic medical records (EMR) systems is preferred
