Provides comprehensive primary care through telemedicine care with a primary focus on older adults, addressing the unique medical, functional, cognitive, and psychosocial needs of the geriatric population. Evaluates, diagnoses, and manages a wide range of acute and chronic medical conditions, including patients with multiple comorbidities, polypharmacy, frailty, dementia, and other age-related health concerns. Delivers evidence-based, patient-centered care through virtual consultations, ensuring continuity of care, medication optimization, preventive care, and chronic disease management. Applies population health and value-based care principles to improve clinical outcomes, optimize healthcare resources, and deliver safe, efficient, and high-quality virtual care for the aging population.

Patient Consultations and Management

  • Deliver comprehensive telemedicine care for family medicine, adults and pediatrics with focus on older adults through virtual assessment, diagnosis, treatment, and follow-up of acute and chronic medical conditions.
  • Conduct comprehensive clinical evaluations, including history taking, virtual physical assessment, medication review, and geriatric assessment where appropriate.
  • Develop and implement individualized, evidence-based management plans, including medication optimization, preventive care, lifestyle interventions, and referrals when indicated.
  • Manage complex geriatric conditions, including multimorbidity, frailty, cognitive impairment, and polypharmacy, ensuring continuity and coordination of care.
  • Collaborate with multidisciplinary healthcare teams, caregivers, and community services to deliver integrated, patient-centred care.
  • Utilize digital health technologies to provide safe, high-quality virtual consultations while maintaining accurate and timely clinical documentation.
  • Promote healthy aging through preventive care, chronic disease management, population health initiatives, and value-based care principles to optimize patient outcomes and healthcare resource utilization

Quality Assurance and Compliance

  • Communicate effectively with patients, families, and caregivers, providing clear guidance on diagnoses, treatment plans, and follow-up care to support informed decision-making.
  • Ensure compliance with clinical, ethical, regulatory, and telemedicine standards while maintaining accurate, timely, and complete medical documentation.
  • Maintain current knowledge of evidence-based practice, telemedicine regulations, and geriatric care through continuous professional development.
  • Contribute to clinical governance by participating in quality improvement initiatives, clinical audits, research activities, and regulatory compliance programs to enhance patient safety and care quality

Professional Development

  • Maintain and continuously enhance clinical knowledge in family medicine with a focus on geriatric care through ongoing professional development and lifelong learning.
  • Participate in relevant conferences, workshops, and continuing medical education (CME) activities to remain current with evidence-based practices and emerging advancements in geriatric medicine and telemedicine.
  • Integrate the latest clinical evidence and digital health innovations into practice to continuously improve the quality, safety, and effectiveness of patient care

Telehealth Platform Proficiency

  • Maintain accurate, complete, and timely clinical documentation in the electronic health record (EHR) in compliance with organizational policies and applicable data privacy regulations.
  • Demonstrate proficiency in telemedicine platforms and digital health technologies, including video consultations, secure communication, and EHR systems, to ensure efficient and high-quality virtual care.
  • Participate in ongoing training and adopt platform enhancements and digital health best practices, collaborating with relevant support teams to optimize system performance and user experience.

Patient Safety and Satisfaction

  • Uphold the highest standards of patient safety by delivering safe, evidence-based care and adhering to established clinical safety protocols across all patient interactions.
  • Identify, report, and escalate patient safety risks, incidents, and quality concerns in accordance with organizational policies.
  • Counsel patients and caregivers on diagnoses, treatment plans, medication use, potential adverse effects, and follow-up care to support informed decision-making and medication safety.
  • Deliver compassionate, patient centered consultations through effective communication, comprehensive clinical assessment, empathy, and shared decision-making to enhance the patient experience.
  • Monitor patient feedback and contribute to quality improvement initiatives that strengthen patient safety, clinical outcomes, and service excellence

Collaboration with Healthcare Teams

  • Collaborate effectively with multidisciplinary teams to deliver coordinated, patient centered care and optimize clinical outcomes.
  • Provide clinical expertise and recommendations to support evidence-based decision-making and integrated care planning.
  • Facilitate clear and timely communication among healthcare professionals to ensure continuity and quality of care.
  • Mentor and support junior physicians and healthcare professionals through clinical guidance, knowledge sharing, and professional development

Skills

  • Medical degree (MBBS, MD or equivalent) from an accredited medical school.
  • Residency training in Family Medicine from an accredited program.
  • Board certification in Family Medicine
  • Fellowship certification in Diabetology.
  • Saudi Commission for Health Specialties SCFHS classification and registration as Family Medicine- Diabetology Consultant.
  • Proven experience as a Family Medicine Diabetology Consultant or similar role, preferably in a hospital or clinical setting.
  • Experience in practicing population health management and value-based care approaches.
  • Proficiency in using electronic medical records (EMR) and telemedicine platforms.
  • Strong clinical judgment and analytical skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport and establish effective relationships with patients and colleagues.
  • Ability to work effectively in a fast-paced environment and manage multiple priorities.
  • Commitment to ongoing professional development and staying abreast of the latest advancements in family medicine with a focus on diabetology, population health management, and value-based care
