Job Purpose:
The Case Manager is responsible to coordinate care of patient undergoing surgeries and patient who are admitted and those planed for discharge ensuring the optimum quality of care and patient satisfaction.
Key Responsibilities And Duties
- Ensures patients are adequately prepared for surgery by verifying consent forms, confirming pre-anesthetic assessments, and educating them about the surgical process via direct communication and SMS.
- Provides information about the surgical process and pre-operative preparation to patients and families.
- Conducts comprehensive assessments of patients scheduled for surgery, including medical history, current medications, allergies, and psychosocial factors, and ensure that relevant steps are in place.
- Discusses management plans with the treating physician and ensure that proper booking and diagnostic tests are completed promptly.
- Collaborates with surgical teams, anesthesiologists, and nursing staff to develop and implement individualized perioperative care plans. Ensures that additional referrals, extra payments, and device availability are managed to avoid cancellations.
- Maintains detailed records of patient interactions, care plans, and outcomes to ensure compliance with Fakeeh Care policies and regulatory requirements.
- If delays occur in the operating room, actively resolve the issues by communicating with the concerned parties.
- Evaluates patients thoroughly upon admission, using referrals and unit rounds to identify their needs and determine care requirements for effective care coordination.
- Facilitates regular meetings with healthcare providers from various disciplines (e.g., nursing, pharmacy, social work) to discuss patient care plans, progress, and discharge needs.
- Creates and adjusts individualized care plans based on ongoing assessments and patient feedback, ensuring all aspects of patient care are addressed.
- Starts discharge planning at the time of admission by evaluating potential post-place needs and facilitating a smooth transition to suitable care settings.
- Identifies and coordinates patient resources, including rehabilitation services, home health care, and outpatient follow-up appointments.
- Acts as a patient advocate by addressing concerns, preferences, and barriers to care throughout the place stay.
- Evaluate patients’ needs for discharge by considering medical, functional, and social factors that may impact their post-discharge care.
- Collaborates with healthcare providers to coordinate follow-up care and ensure all necessary services are arranged before discharge.
- Identifies potential barriers to safe discharge, such as lack of transportation, financial constraints, or insufficient home support and work to address these issues.
- Connects patients and families to community resources, support services, and financial assistance programs as needed.
- Participates in quality improvement initiatives to enhance the pre-operation process and discharge planning.
- Promotes patient-centered care by ensuring that all care activities are focused on the patient’s needs and preferences.

- Completion of mandatory education as per the requirement, at least one month prior to expiration.

Experience
Minimum of 2 years in the same filed
Education
Bachelor of Science in Nursing Degree, or MBBS
