Job Description
The Medical Transcriptionist is responsible for accurately transcribing, reviewing, and editing medical reports, physician dictations, patient histories, discharge summaries, consultation notes, diagnostic results, and other healthcare documentation. The role ensures that all medical records are complete, accurate, confidential, and maintained in accordance with healthcare regulations and organizational standards.
Key Responsibilities
Medical Documentation
- Transcribe audio recordings and dictated medical reports into written documents.
- Prepare, edit, proofread, and format medical records for accuracy and completeness.
- Review transcribed reports for consistency, grammar, spelling, and medical terminology.
- Ensure all documentation is completed within established turnaround times.
- Update and maintain patient records in electronic medical record (EMR) systems.
Quality Assurance
- Verify the accuracy of patient demographics, diagnoses, treatments, and procedures.
- Identify and clarify inconsistencies, omissions, or discrepancies in dictated reports.
- Maintain high standards of transcription quality and documentation accuracy.
- Follow established medical documentation guidelines and quality standards.
Records Management
- Organize and archive medical documents in accordance with healthcare policies.
- Maintain confidentiality and security of patient information at all times.
- Ensure records comply with applicable healthcare regulations and privacy requirements.
- Assist healthcare professionals in retrieving and managing medical documentation when required.
Communication and Coordination
- Liaise with physicians, nurses, and other healthcare professionals to clarify unclear dictations.
- Coordinate with medical records and administrative teams to ensure accurate documentation workflows.
- Provide support during audits and quality reviews of medical records.
Qualifications
- Diploma or Bachelor’s degree in Medical Transcription, Health Information Management, Medical Administration, or a related field.
- Certification in Medical Transcription or Healthcare Documentation is an advantage.
Experience
- Minimum 1-3 years of experience in medical transcription or healthcare documentation.
- Experience working in hospitals, clinics, diagnostic centers, or healthcare facilities is preferred.
Knowledge and Skills
- Strong knowledge of medical terminology, anatomy, physiology, pharmacology, and healthcare practices.
- Proficiency in medical transcription software and electronic medical record (EMR) systems.
- Excellent typing speed and accuracy.
- Strong proofreading, editing, and documentation skills.
- Good command of written and spoken English.
- Ability to maintain strict confidentiality of patient information.
- Strong attention to detail and organizational skills.
- Ability to work under pressure and meet deadlines.
