Physical Therapist | Home Healthcare

Provide individualized physical therapy services to patients in their homes, focusing on improving mobility, physical function, independence, and overall quality of life.

Key Responsibilities:

• Conduct comprehensive physical and functional assessments.
• Develop and implement individualized treatment and rehabilitation plans.
• Provide evidence-based therapeutic exercises and physical therapy interventions.
• Improve patients’ strength, flexibility, balance, mobility, and functional abilities.
• Monitor patient progress and adjust treatment plans according to clinical needs.
• Educate patients and caregivers on home exercise programs and safe mobility techniques.
• Work collaboratively with physicians, nurses, and other healthcare professionals to provide holistic patient care.
• Maintain accurate clinical documentation and progress reports.
• Follow healthcare, safety, infection control, and professional standards.
• Provide patient-centered care while maintaining confidentiality and professional ethics.
