Job Description
Job Requirements:
- Bachelor’s degree in pharmacy (BPharm) or Doctor of Pharmacy (PharmD) from an accredited institution.
- Valid SCFHS license as Specialist.
- Minimum of 3 years of pharmacy practice experience, preferably in long-term care, rehabilitation, or similar healthcare settings.
- Experience working with elderly populations and managing complex medication regimens.
- Familiarity with LTC regulations, medication safety protocols, and interdisciplinary team collaboration.
Key Job Responsibilities:
- Review and approve medication orders for residents.
- Perform comprehensive medication reconciliation during admission/discharge.
- Educate residents and caregivers on medication use.
- Monitor residents for therapeutic effectiveness and adverse effects.
- Collaborate with healthcare providers to optimize medication therapy.
- Maintain accurate medication records and documentation.
- Ensure medication storage complies with safety standards.
- Manage inventory to prevent shortages or excess.
- Lead staff training on medication safety protocols.
- Participate in multidisciplinary care planning.
- Conduct audits and quality assurance activities.
- Report medication incidents or adverse events.
- Keep updated with current pharmacological research and guidelines.
- Support infection control and safety measures related to medication use.
- Facilitate medication administration procedures in accordance with best practices.
