Job Description

Job Requirements:

  • Bachelor’s degree in pharmacy (BPharm) or Doctor of Pharmacy (PharmD) from an accredited institution.
  • Valid SCFHS license as Specialist.
  • Minimum of 3 years of pharmacy practice experience, preferably in long-term care, rehabilitation, or similar healthcare settings.
  • Experience working with elderly populations and managing complex medication regimens.
  • Familiarity with LTC regulations, medication safety protocols, and interdisciplinary team collaboration.

Key Job Responsibilities:

  • Review and approve medication orders for residents.
  • Perform comprehensive medication reconciliation during admission/discharge.
  • Educate residents and caregivers on medication use.
  • Monitor residents for therapeutic effectiveness and adverse effects.
  • Collaborate with healthcare providers to optimize medication therapy.
  • Maintain accurate medication records and documentation.
  • Ensure medication storage complies with safety standards.
  • Manage inventory to prevent shortages or excess.
  • Lead staff training on medication safety protocols.
  • Participate in multidisciplinary care planning.
  • Conduct audits and quality assurance activities.
  • Report medication incidents or adverse events.
  • Keep updated with current pharmacological research and guidelines.
  • Support infection control and safety measures related to medication use.
  • Facilitate medication administration procedures in accordance with best practices.
