Advanced Technical & Core Bench Responsibilities
·  Grossing Assistance: Assist the consultant pathologists during the macroscopic examination and grossing of complex surgical specimens and tumor resections.

· Precision Microtomy: Execute high-precision sectioning using rotary microtomes, cutting ultra-thin tissue sections (3 to 5 microns) without artifacts, which is critical for accurate cancer staging and diagnosis.

· Frozen Sections: Independently run and manage the cryostat machine for ultra-fast frozen section testing required during live, intraoperative surgical consultations

·  Special Staining & Advanced Assays: Oversee routine Hematoxylin and Eosin (H&E) setups, while independently executing specialized chemical stains (e.g., PAS, Giemsa, Reticulin) and assisting with advanced Immunohistochemistry (IHC) biomarker protocols.

Quality Assurance & Regulatory Compliance (NABL / JCI)
·  Quality Control (IQC & EQAS): Document, monitor, and sign off on internal quality controls for automated tissue processors and slide stainers. Participate closely in External Quality Assurance Schemes.

·  SOP Enforcement: Maintain and update standard operating procedures (SOPs) to ensure strict adherence to safety, environmental, and biosafety protocols.

· Error Prevention: Supervise pre-analytical documentation to eliminate critical errors in patient block labelling, specimen identification, and data logging within the Laboratory Information Management System (LIMS).

Laboratory Management & Supervision
· Junior Staff Mentorship: Supervise, train, and guide junior lab technicians, trainees, and laboratory assistants on complex microtomy techniques and tissue handling safety.

·  Equipment Upkeep: Oversee the calibration, maintenance, daily cleaning, and troubleshooting of specialized machinery (e.g., automated tissue processors, embedding stations, automatic stainers, and microtome blades).

· Inventory Control: Manage the stock and requisition matrices for critical, high-risk laboratory chemicals (like formalin, xylene, and paraffin wax), ensuring zero downtime.

Critical Reporting & Turnaround Time (TAT)
· TAT Management: Manage the structural workflow of the surgical pathology block to ensure slides reach the Pathologist’s desk within the strictly defined clinical turnaround times.

· Archiving Safeguards: Oversee the safe, systematic, long-term physical and digital archiving of patient tissue blocks and corresponding glass slides for historical reference or secondary consultations.

As TECHNICIAN
Advanced Core Technical Duties
· Complex Procedure Execution: Performing advanced diagnostic tasks autonomously (e.g., precise microtomy sectioning at 3-5 microns, handling specialized immunohistochemistry (IHC) markers, or validating complex immunoassay controls).

· Intraoperative Support: Managing ultra-fast diagnostic setups like the Cryostat machine for frozen section testing during active surgeries—a task strictly reserved for trusted, higher-grade technicians.

· Grossing & Pathology Assistance: Providing structural macro-examination and grossing assistance directly to Consultant Pathologists on intricate surgical and biopsy specimens.

Rigorous Quality Control (NABL & JCI Mandates)
· IQC & EQAS Monitoring: Running, recording, and charting daily Internal Quality Controls. A PM4 technician is responsible for identifying calibration shifts, documenting corrective and preventive actions (CAPA), and handling External Quality Assurance Scheme evaluations.

· Critical Alerts: Validating and immediately escalating critical “panic values” to clinical wards and physicians, strictly maintaining the hospital’s Turnaround Time (TAT) logs.

· Audit Compliance: Ensuring the workstation complies fully with national (NABL) and international (JCI) accreditation benchmarks, including maintenance of equipment logs and temperature charts.

Equipment & Systems Management
·  Automation Oversight: Calibrating, operating, and troubleshooting advanced laboratory systems (e.g., automated tissue processors, automated slide stainers, and advanced chemistry/hematology analyzers).

· Informatics Mastery: Efficiently managing sample data tracking, pre-analytical barcoding audits, and validation releases through the hospital’s Laboratory Information Management System (LIMS).

Leadership & Operational Support

· Mentorship: Guiding, supervising, and training junior technicians (PM1–PM3 bands), interns, and lab assistants on proper laboratory techniques and safety guidelines.

· Inventory Ownership: Monitoring the buffer stock, checking expiration dates, and drafting requisition orders for high-stakes laboratory reagents and hazardous chemicals.

· Bio-Safety Enforcement: Maintaining absolute compliance with infection control and biomedical waste disposal policies to guarantee a biohazard-free working space.

As JUNIOR LAB TECHNICIAN
Specimen Receiving, Verification & Grossing Assistance
· Accessioning: Receive tissue specimens (biopsies, large resections, fluid cytology) from the O.T. or wards. Verify that the patient’s details on the vial match the test requisition form (TRF) exactly.

· Fixation Check: Ensure the tissue is correctly immersed in the proper volume of fixative.

· Grossing Assistance: Assist the Pathologist or Senior Technician during gross examination by numbering cassettes, preparing measurement tools, and accurately documenting descriptions.

Tissue Processing & Embedding
· Processor Management: Load tissue cassettes into automated tissue processors and ensure the sequence of dehydration (alcohol grades), clearing (xylene), and wax infiltration (paraffin) is correctly set.

· Paraffin Embedding: Transfer processed tissue into metal moulds and orient the tissue correctly in molten paraffin wax to create blocks. Correct spatial orientation is crucial so the pathologist cuts through the correct plane of interest.

Microtomy (Tissue Sectioning)
· Section Cutting: Use a rotary microtome to slice embedded tissue blocks into ultra-thin sections.

· Ribbon Floating & Mounting: Float the fragile tissue ribbons onto a warm water bath to remove wrinkles, then carefully mount them onto clean glass slides.

Staining & Cover slipping
· Routine Staining: Perform routine Haematoxylin and Eosin (H&E) staining using manual or automated methods.

· Special Stains: Assist or perform special histochemical stains (e.g., PAS, Reticulin, Masson’s Trichrome, AFB) as guided by senior staff to highlight specific cell components or organisms.

· Cover slipping: Apply mounting media (like DPX) and coverslips over stained sections neatly, ensuring there are no air bubbles that could block microscopic viewing.

Quality Control & Maintenance
· Daily QC Check: Run daily control slides alongside patient samples to verify staining accuracy and chemical efficacy.

· Equipment Maintenance: Clean and maintain lab equipment daily, including microtome blade adjustments, water bath temperature monitoring, and routine cleaning of tissue processors.

· Reagent Log: Track and rotate reagents (handling hazardous chemicals like Xylene, Formalin, and absolute alcohols safely) and document batch changes.

Archiving & Biomedical Waste Management
· Filing & Retrieval: Systematically file and index paraffin blocks and finished slides for long-term storage, enabling rapid retrieval if a case needs a review or immunohistochemistry (IHC) tests.

· BMW Disposal: Sort and dispose of chemical wastes and sharp items (used microtome blades) according to strict institutional Biomedical Waste Management guidelines.
