As a Medical Social Worker, I provide psychosocial assessment, counselling, and support to patients and their families while working collaboratively with a multidisciplinary team. I assess patients’ social, emotional, and psychological needs and provide appropriate interventions to support coping, treatment adherence, lifestyle management, and overall quality of life. I maintain accurate and confidential documentation, monitor patients’ progress, and contribute to patient-centered care and improved patient experience. I also support health promotion, discharge planning, quality improvement, and social care development while following professional standards, organizational policies, and healthcare regulations.
